As we mentioned earlier, we found out very early on in the game that Sam actually has Aphenphosmphobia. Why he has this phobia remains to be seen, but the definition above certainly gives us an idea.

It could be that Sam is just constantly worried about becoming contaminated or falling ill. With the dead turning into BTs themselves when they die, and then opening up the possibility of a void-outs happening.

These are essentially massive explosions that can level entire cities if theyre within proximity of the explosion, and is basically how the USA fell into the state that we find it at the beginning of the game.

As such, it kinda makes sense that Sam would be incredibly cautious of people touching him, in case they give him some sort of disease and he ends up dying and turning into a BT. It could also just be a general fear of being touched thanks to the BT handprints that indicate one of the big creepy ghost things is nearby.

In season 3, Episode 28 of Dan Foglers TV & Film Podcast, the actor talked a bit about the game and his experiences working on it, and revealed some interesting details. And, you know, Guillermo del Toro, he just wrote me and he well, he and Hideo and I were supposed to do a different game, and that game kinda fell apart because Hideo and that company kinda fell apart , they kinda broke up in Japan and Sony picked up Hideo and he started his own thing at Sony.

Norman was then asked what the theme of the game was, and what kind of character he plays. He provided an interesting answer.

The host then asked when the game is going to be available, to which Norman responded: I think its going to be early this coming year. But I think I have two more no I have three more sessions here in New York starting next week.

As you can see Norman believes the game is going to come out early next year, but this seems rather unlikely given the fact that it doesnt even have a release date yet.

A more accurate condition for Sam, then, which would explain the physical impact it has on him, could be tactile allodynia. That’s a neurological condition that can make touching physically painful, as well as other normal stimuli like temperature. But that’s less of a nice metaphor.
