Along with genetics, other potential causes of schizophrenia include:

  • The environment. Being exposed to viruses or toxins, or experiencing malnutrition before birth, can increase the risk of schizophrenia.
  • Brain chemistry. Issues with brain chemicals, such as the neurotransmitters dopamine and glutamate, may contribute to schizophrenia.
  • Substance use. Teen and young adult use of mind-altering drugs may increase the risk of schizophrenia.
  • Immune system activation. Schizophrenia can also be connected to autoimmune diseases or inflammation.

The Effects Of Stress

Many psychiatric disorders, including schizophrenia, are associated with disturbances of the bodys stress responses. This is reflected in levels of cortisol, which is a steroid hormone thats produced by the adrenal gland in stressful situations.

Chronic production of high levels of cortisol has a damaging effect on virtually all body systems, including the brain.

It is possible, and even probable, that belonging to a small migrant group is associated with a state of chronic stress, producing increased baseline cortisol and therefore increased risk of schizophrenia.

There are reasons to believe that this kind of stress effect is more powerful in urban areas, because city living affects the brains response to stress. Baseline levels of crime, social fragmentation and urban decay are also important.

This model, linking community factors with effects on individual brains, receives strong support in a study by Duke University and Kings College London, published in Schizophrenia Bulletin in May in 2016.

The team analysed data from over 2,000 UK-born twins and found that reduced social cohesion and crime victimisation likely explain, at least in part, why children in cities have an increased risk of developing symptoms of disorders such as schizophrenia. It is not the cities themselves, then, but the way we live in them that likely matters most.

Individual History: Alexander Lee Skip Spence was a Canadian-born American musician and singer-songwriter. He began his career as a guitarist in an early line-up of Quicksilver Messenger Service, and was the drummer on Jefferson Airplanes debut album, Jefferson Airplane Takes Off. He was one of the co-founders of Moby Grape, and played guitar with the group until 1969. He released one solo album, Oar, and then withdrew from the music industry. His career was plagued by drug addiction and mental health problems. In the late 1960s, Spence became delusional after taking too much acid. He was found marching around his hotel room with an axe, bashing down doors and claiming he was the anti-Christ. At that point he was taken to Bellvue Hospital where he was formally diagnosed as schizophrenic. Spence died in 1999 from lung cancer, two days before his 53rd birthday.

    Defining The Prevalence Of The Disease

    The prevalence rates of schizophrenia depend upon a whole range of factors, such as the availability of and response to treatment. The prevalence of schizophrenia, as with other mental disorders, can be calculated from a number of sourcesfrom case registers to field surveys. The latter lend themselves more readily to estimation of period prevalence than point prevalence, while case register data can provide point prevalence more readily. The denominator can be the whole population or only a small defined population.

    Saha et al. quite rightly differentiate between traditional prevalence, or core, studies , and studies in specific sub-groups . Using sequential filters they were able to isolate discrete data from multiple studies, and they used other strategies to ensure that the largest groups were counted.

    The authors had hypothesised that prevalence estimates would differ between lifetime, period, and point prevalence and that lifetime prevalence would be higher than lifetime estimates . They also predicted that the estimates would be higher for males, those from urban areas, and migrants. Case ascertainment methods and sample selection do influence prevalence rates, so Saha et al. chose studies that used comprehensive case ascertainment methods.

    Social Isolation A Major Problem

    While these psychotic symptoms are more alarming, other symptoms reinforce the alienation of people with schizophrenia. They are often unable to participate in normal social events or conversations, and lack sufficient motivation for simple activities like bathing or cooking. In addition, sufferers lack the insight to recognise how their inappropriate behaviour appears to others.

    A Future After Schizophrenia

    Treatments And Risk Factors

    There are many pharmaceutical, psychological and social treatments for schizophrenia, and these help patients and families a great deal. It is critical that these treatments are delivered efficiently, effectively and with compassion, to heal and empower the mentally ill and their families.

    But these treatments are deeply imperfect and are not cures for schizophrenia.

    The prospects of better treatment would be much improved if we understood precisely what causes schizophrenia in the first place. But we do not.

    In the search for answers, it is critical to develop a better understanding of urbanicity and even more so its relationship with other risk factors, such as genes, prenatal or birth injury, psychological trauma, cannabis, head trauma, migration, social adversity, chronic stress and others.

    All are of these linked with schizophrenia to varying degrees, but none are fully understood.

    Ultimately, research is hampered by the fact that schizophrenia is defined by symptoms rather than biological tests. Schizophrenia, like fever or headaches, is almost certainly an umbrella term that covers a family of different but related sub-disorders, rather than a single, biologically distinct entity.

    These sub-disorders, despite sharing many symptoms, might well have somewhat different origins in different groups or individuals. As a result, schizophrenia retains the ultimate mystery that is intrinsic to all true scientific enigmas: it might not exist as a definable entity.

    Public Opinion / Perception Of Schizophrenia

    Worldwide, people diagnosed with schizophrenia face stigma and discrimination. The stigma associated with schizophrenia can delay a diagnosis as many people may hesitate to discuss their symptoms even with a healthcare provider.     Depression Is Complicated This Is How Our Understanding Of The Condition Has Evolved Over Time

    People often think of depression as a single, uniform condition deep sadness and a loss of interest in the activities that someone usually enjoys. But depression is complicated and its difficult to define what it means in an objective way.

    This is because depression is a condition of the mind: it is diagnosed based on peoples psychological symptoms and behavior, not from a brain scan or markers in their blood or DNA.

    When we diagnose depression, we have to rely on people to recall their own symptoms. We have to trust that doctors will observe and probe their symptoms reliably. We have to analyze this information to understand what people with depression experience that other people dont.

    Our ability to do all of these things accurately has changed over time, and so has our understanding of depression.

    This comes down to three factors.

    First, many countries now screen for depression in the general population, not just in a subset of people who are seen by a small number of doctors. In many studies, researchers track patients over long periods of time to understand how the condition progresses.

    Second, we use questionnaires and interviews that treat depression as a condition that can occur at different levels and change with time.

    Third, we have better statistical tools to help us refine questionnaires and understand how symptoms are related to one another.
