Its a fact that there is a small subset of kids with ADHD who get moody and irritable when they take stimulant medications. It usually happens right away, as soon as they start taking the medication, and goes away immediately when they stop taking it. In my experience, about 1 in 25 kids have this reaction. We usually recommend trying some different stimulants if this happens, because some kids react differently to those based on methylphenidate and those based on amphetamine. But it sounds like youve tried that. If stimulants do not work or cannot be tolerated, there are non-stimulant medications for ADHD that should be considered. You can read about them here.
Some children have depression along with having ADHD, and for those kids it is often effective to treat the ADHD with stimulants and the mood disorder with an antidepressant, if psychotherapy is not enough. The combination of these two types of medications isnt a problem for them. In general, however, we try to avoid the practice of adding medications to just to deal with side effects, and I dont even know if an antidepressant would work to reverse sadness or irritability created solely as a side effect of stimulants.

Side Effects Of Antidepressants

Vyvanse comes as a chewable tablet and a capsule. People who have trouble swallowing can open the capsule and mix the powder inside with yogurt, water, or orange juice.
Concerta comes as an extended-release tablet. This must be swallowed whole.
Personal Experience Taking Vyvanse For Depression
For several months, I took Vyvanse on an intermittent basis to treat depression. Although it was meant to be used as an adjunct, I used it intermittently as a standalone option. I wasnt too keen on continuing with my antidepressant due to undesirable side effects, so I stopped it and only used Vyvanse as needed.
Due to the potential for abuse, I used an extremely low dose by taking a 30 mg capsule and pouring the powder into yogurt this was recommended by my psychiatrist. I probably was taking somewhere around 10 mg to 15 mg and experienced noticeable benefit. It may not have made me feel happy or euphoric like Adderall did, but it certainly alleviated my depression.
It dramatically increased my mental clarity, optimism, and cognitive function. Despite my experience with low dose Vyvanse for depression, I never wanted to become dependent upon it or fall victim to the long-term trap of downregulation of dopamine receptors and low dopamine levels. That said, nearly every time I took it, it eradicated my suicidal thoughts and depression, while simultaneously giving me hope for the future.

Treating Major Depression And Adhd
What should people with both ADHD and MDD do? Which should be addressed first? The decision is usually made by the patient based on what he thinks is the most urgent or impairing condition. Given the choice, I treat ADHD first with a stimulant. This is based on my experience that a high percentage of patients report that their mood lifts when they have achieved optimal doses of stimulant-class medication.
If the depressive symptoms persist, an antidepressant is usually added to the ADHD medication. Many clinicians opt for fluoxetine , since it has no effect on ADHD and its long duration in the body makes it an ideal drug for patients who forget to take it.
Some clinicians may use a second-line medication alone for cases of mild to moderate depression plus ADHD. It should be noted that, while antidepressant medications have published studies to show that they help with ADHD symptoms, none have shown robust effects. They have demonstrated detectable benefits but only as second-line medications when the use of stimulants or an alpha agonist is not appropriate.

How Should Patients With Degenerative Cervical Myelopathy Be Evaluated And Treated

This drug should not be taken at the same time as any monoamine oxidase inhibitors . In addition, you should wait 14 days between discontinuing an MAOI and starting to take Vyvanse, so that the MAOI has completely worked out of your system. Taking the two drugs together increases the risk of a dangerous hypertensive crisis.
Other problematic medications commonly used to treat bipolar disorder may include:
- Tricyclic antidepressants, especially Norpramin and Vivactil , which may cause a significant increase in the amount of Vyvanse in the body
- Haldol , which may make Vyvanse less effective
- Lithium, which may also make Vyvanse less effective
How Long Does Vyvanse Last In Your System
It typically takes about 2 to 3 days before Vyvanse is fully cleared from your system. However, this may vary depending on your age, weight, genetics, kidney function, or other medications you may be taking.
The effects of Vyvanse typically last for about 10 to 12 hours after you take a dose. After the effects of Vyvanse have worn off, the levels of Vyvanse in your body continue to decrease.
Vyvanse has a half-life of about 12 hours. A drugs half-life is the amount of time it takes for your body to remove half a dose of medication from your system. After you take a dose of Vyvanse, it takes about 12 hours for the amount of active drug in your body to decrease by half.

When I am working with a new patient in the Seattle or Portland area to find the right treatment for their anxiety, depression, or ADHD, they sometimes have concerns. These new patients sometimes voice questions about whether or not the medication I prescribe for them can be addictive. Some might be concerned that they will become dependent on their anti-anxiety medication. Or, they might have heard that stimulants used to treat adult Attention Deficit Hyperactivity Disorder are likely to be misused.
Different classes of medications have differing levels of possibility for abuse and dependence. Some types of medication that treat anxiety are addictive, and others have not been shown to be. The same is true for ADHD medication.
Another consideration is that when patients have a behavioral health condition like anxiety or ADHD that isn’t being treated, they might use marijuana or alcohol to self-medicate. Although people might be trying to relieve their symptoms, using alcohol or marijuana to treat your behavioral health symptoms can come with a whole host of other problems.

Despite the fact that formal FDA approval wasnt granted to Vyvanse as an adjunct treatment for major depression, there is evidence suggesting that it may be helpful for select individuals particularly those with treatment-resistant depression. Additionally, those that have comorbidities of ADHD, binge eating disorder, executive dysfunction, or fatigue may derive significant benefit from Vyvanse. Certainly this drug shouldnt be used in everyone, particularly those with addictive personalities.
That said, if a psychostimulant is to be used as an adjunct, Vyvanse may be a preferred option over older drugs. This is due to the fact that it is a prodrug, is believed to have a smoother absorption, is less likely to make a person crash, and has a long duration of effect. However, some psychiatrists may still prefer Provigil over Schedule II substances like Vyvanse due to its well established efficacy and reduced abuse potential.
